The idea is, you join in, and you get placed at the bottom of the list to start out, but when someone signs up after you, they have to put a link for your site on theirs. This follows all the way back to the top. What you do is, you put a small box full of links at the bottom of an commonly visited page, and then your link gets displayed on anyones page under you. Also, you can go up in the list by making donations. Your total amount donated, how long you have been with the site, and overall quality of page will increase your rank on the list, therefore driving more and more people to see your link, and visit your site.
